Grown-Up Acne: Why Am I Still Getting Pimples?

Acne is not just a teenage problem. Many people continue to have acne in their 20s, 30s, 40s and sometimes even later. Some people may even develop acne for the first time as an adult.

A. Why does acne happen in adults?

Adult acne can have several causes. These include oily skin, blocked pores, inflammation, genetics, stress, certain medications and hormonal changes.

Hormones are particularly important. In women, acne may change with the menstrual cycle or be associated with conditions such as polycystic ovary syndrome (PCOS). Acne around the chin and jawline can sometimes have a hormonal component.

Hormones can also contribute to acne in men, particularly when testosterone or other hormonal treatments or supplements are being used.

B. What is the best way to diagnose adult acne?

The first step is a complete skin assessment by a trained skin clinician. Not every red spot or breakout is acne, and identifying the type and possible cause is important before starting treatment.

We may also use VISIA skin analysis to look more closely at oiliness, pores, redness, pigmentation and other features of the skin. In some patients, particularly where a hormonal cause is suspected, further assessment or blood tests may also be appropriate.

C. What is the best treatment?

There is no single treatment that works for everyone. Treatment should be selected according to the type of acne, its severity, the underlying cause and whether scarring or pigmentation is already present.

Treatment may include:

  • Topical treatments such as prescription creams or gels.

  • Oral medications when appropriate, particularly for persistent or more severe acne.

  • Hormonal treatment in selected patients where hormones are contributing to the acne.

  • Laser and other skin treatments for selected cases.

  • Acne scar treatment, including laser treatments to improve the appearance and texture of acne scars once the active acne is under control.

The important point is to treat both the active acne and its underlying cause, rather than simply treating individual pimples as they appear.
